The processing and application fee of Rs. 2,000 should be submitted via Online Intra Bank Transaction in favor of Deputy Dean Shaikh Zayed Postgraduate Medical Institute, Lahore, Account No. 3154820902 at the National Bank of Pakistan (NBP), Shaikh Zayed Hospital, Lahore, Branch code 1707. Forms can be sent via courier to the address provided on the form.
- A copy of the online Intra Bank Transaction receipt must be attached to the application form. Forms are also available from the account section of FPGMI, Lahore, upon payment of Rs. 2,000.
- Incomplete applications will be rejected. The last date for submission of application forms is July 4, 2024.
